Organ Donation Form Software
Use this form to state your intent to donate organs and tissues. You may also instruct that your donation be used for such purposes as transplantation, education, or research.
Organ donations are often referred to as "anatomical gifts". All fifty states have passed some version of the Uniform Anatomical Gift Act, generally providing that you may make a gift of your organs and tissues during your lifetime with the gift to take effect on your death. Additionally, family members may generally donate your organs and tissues after your death if there is no indication you were opposed to such a donation. It is therefore important for you to document your wishes concerning organ donation.
