Dog Walking example Form Software
In this example a Dog Walker agrees to provide dog walking services for a business or an individual who is the Owner of the Dog. The Dog Walker is not an employee of the business or individual that is contracting for services but rather an independent contractor.
The Dog Walking example is a document under which a "Dog Walker" contracts to provide services for a "Dog Owner." Under this example, the Dog Walker is an independent contractor with respect to the Dog Owner, and not an employee of the Dog Owner. This example should not be used if the Dog Walker is really an employee of the Dog Owner.
Through a Dog Walking example you can engage a Dog Walker to walk your dog. A dog walking service might be preferred when the Owner is unable to dedicate the time needed to regularly walk and exercise his or her animal.. With this example you can specify the terms of the dog walking service. For example, on which days the dog should be walked, at what hour of the day, and the duration of each walk amongst other things.
The following documents are included as attachments to this example:
Owner's Information Sheet (Exhibit A): This attachment included contact information for the Dog Owner.
Pet Information Sheet (Exhibit B): This attachment includes identifying information about the dog and any important history information the Dog Walker should know such as habits, allergies or other important information.
Veterinary Release Form (Exhibit C): This attachment includes a release for the Dog Walker to request veterinary treatment for the dog should it be required.
The Independent Contractor example should be signed by both parties and becomes effective as of the date inserted at the beginning of the example.
